What can cause fertility problems for men? The most common cause of infertility in men is sperm problems. This could be any combination of the following: a low sperm count, or no sperm at all abnormally shaped sperm, which are less able to move and fertilise an egg sperm not moving well (poor motility) Other causes of fertility problems in men include: ejaculation problems, such as delayed ejaculation or retrograde ejaculation (where sperm moves backwards into the bladder) problems with the testicles, for example following infection, surgery or injury low levels of male hormones (hypogonadism) Possible symptoms The only way to tell if you have a sperm problem is to get your sperm tested. Your GP should offer you a test if you've been trying to conceive for a year or more (or sooner, if your partner's over 35). You'll probably know if you have an ejaculation problem, because either no sperm, or very little sperm, will come out of your penis during sex or masturbation. If there's a problem with your testicles, you may feel pain or discomfort, and you or your GP may notice a lump in or around your testicles. If you have low levels of male hormones, you may have a low libido, difficulties getting or maintaining an erection, and experience hot flushes. Possible solutions If you have sperm problems because of low levels of certain hormones, fertility medication may help you to conceive. Ejaculation problems can sometimes be treated with medication or lifestyle changes, sex therapy, or treatment for an underlying illness, such as diabetes. If a blockage in your testicles is affecting your fertility, it may be possible to remove it surgically. If other treatments don't help, you may still be able to have a baby by in-vitro fertilisation (IVF), probably using a process where sperm is injected directly into the egg (intracytoplasmic sperm injection, or ICSI). If you don't have any healthy sperm, or there's a high risk of you passing on an inherited health condition, donor sperm may be your best chance of starting a family.
